Girl Scout Permission & Health History Form (updated 2025)
This should be on-hand for all troop meetings, trips, events & activities. Keep them in a binder or folder for easy access when needed. Caregivers should review and update annually.
As of 2025, there is a single form for youth and adult Girl Scouts. Troop leadership should have all registered adults complete these forms and store them with the youth forms.
Parent/Guardian Permission Form for Outdoor Overnights, High-Risk Activities and Sensitive Topics
Use this form for each:
overnight trip, and/or
troop meeting that includes topics that some scouts or families may be sensitive to, and/or
activity that are considered high-risk per the Safety Activity Checkpoints
Have each adult who will be driving scouts complete this form. It has details you’ll need to include on your TAST (see next section).
Travel Application for Short Trips (TAST Form)
Complete & submit for overnights of 1-2 nights (or 3 nights if includes a Federal holiday). TASTs are required whether overnight is at your home, at a GSWW property, or elsewhere.

TAST Tips & Tricks:
Create a primary troop roster document of all the registered youth & adults in your troop. Include the information required on the TAST’s last page (name, date of birth, emergency contact’s name & number). Then for each TAST submission, rename that document and update it with the planned attendance. Just prior to that trip, make sure it’s accurate and send a copy to your emergency contact at home (also listed on that TAST) so they know who actually attended.
If you are traveling by private car, then you will need information from each driver, including their driver’s license number, insurance carrier and policy number. Use this Transportation Card, and then transfer the information to your TAST.
Troops are required to track and report on their finances each year. The Troop Leader, Assistant Troop Leader, or Treasurer starts with this spreadsheet, which uses the same categories that you will need for the Annual Troop Finance Report (ATFR) that is due to GSWW by June 30th of each year. Save a copy before you enter data. The ATFR is now submitted through the Volunteer Toolkit, which can be accessed by logging into your account via the myGS button at the top. This "Hot Sheet", will help you in completing it.
